The short answer

Ultrasound therapy uses sound waves above the audible range for physical treatment. It is different from diagnostic ultrasound imaging. Its role depends on the diagnosis, settings and treatment plan.

How therapeutic ultrasound works

A treatment head transfers high-frequency mechanical vibrations to a defined area of the body. A coupling medium helps transmit the sound waves. Depending on the settings, thermal and mechanical effects play a role. Continuous and pulsed delivery are different ways of applying the energy.

Pain, mobility and tolerance of activity are different outcomes. A change in one does not, by itself, demonstrate tissue healing. Ask what the proposed treatment aims to achieve and how that outcome will be assessed.

Treatment is different from diagnostic imaging

Diagnostic ultrasound uses returning echoes to create an image. A therapeutic ultrasound device does not automatically show a tendon, tear or joint. If the cause of a complaint is unclear, assessment and diagnosis come before choosing a device.

Ultrasix US and Combisix UE

Sixtus Italia lists Ultrasix US with 1, 2 and 3 MHz and continuous or modulated delivery. Combisix UE combines ultrasound with two-channel electrotherapy. Several functions in one system may suit a practice workflow; they do not prove a better clinical outcome.

What can you reasonably expect?

Discuss how ultrasound fits alongside exercise and everyday activity. Agree when progress will be reviewed and when treatment should be changed or stopped.

Evidence depends on the condition

NICE does not recommend ultrasound therapy for osteoarthritis because there is insufficient evidence of benefit. This recommendation concerns osteoarthritis and should not be extended to every indication.

A functioning device or a pleasant treatment sensation is not proof of clinical benefit.

Frequently asked questions

Is ultrasound therapy the same as an ultrasound scan?

No. Diagnostic ultrasound creates images; therapeutic ultrasound applies sound energy for a treatment goal.

What do 1, 2 and 3 MHz mean?

They describe frequencies, not treatment duration, dose or proof that treatment will work for a particular complaint.

How do Ultrasix US and Combisix UE differ?

Ultrasix US is an ultrasound system. Combisix UE combines ultrasound with two-channel electrotherapy.

Can ultrasound replace exercise?

Its role should be considered within the whole treatment plan. Your clinician determines the appropriate exercise and activity programme.

How many sessions are needed?

There is no universal number. Agree on a goal, a review point and criteria for changing or stopping treatment.

Is ultrasound always useful for osteoarthritis?

No. NICE does not recommend it for osteoarthritis because evidence of benefit is insufficient. Other indications require separate assessment.
